Root Cause Wellness Consultation Terms & Informed Consent

Effective Date: 28/09/2023
Last Updated: 31/07/2026

Welcome

Thank you for choosing Root Cause Wellness.

We are honoured to be part of your health journey.

At Root Cause Wellness, our goal is to help individuals and families better understand their health through education, thoughtful discussion and complementary health support.

We believe informed clients make empowered decisions.

This document explains what you can expect from us, what we ask of you, and how we work together to create the best possible experience.

By booking or attending a consultation with Root Cause Wellness, you acknowledge that you have read, understood and agree to these Consultation Terms and provide your informed consent to receive our services.

1. Nature of Our Consultations

Root Cause Wellness provides complementary health consultations designed to educate and support your overall wellbeing.

Our consultations may include discussion of:

  • Homeopathy
  • Tissue salts
  • Nutrition
  • Lifestyle factors
  • Emotional wellbeing
  • Environmental influences
  • Wellness education
  • General health support
  • Health history
  • Preventative wellbeing strategies

Our consultations are intended to complement—not replace—appropriate medical care.

We encourage clients to maintain a relationship with their GP and other registered healthcare professionals where appropriate.

2. No Medical Diagnosis

Root Cause Wellness does not diagnose medical conditions or provide medical diagnoses.

Our consultations should not be interpreted as medical advice or a substitute for assessment by a registered medical practitioner or other appropriately qualified healthcare professional.

If your symptoms suggest a medical emergency or require urgent assessment, we will recommend that you seek immediate medical care.

3. Scope of Recommendations

Recommendations provided during your consultation are based on:

  • the information you choose to provide;
  • your health goals;
  • your consultation responses; and
  • our professional opinion within the scope of our complementary health practice.

Recommendations are educational in nature and are intended to support informed decision-making.

No recommendation should be interpreted as a guarantee of a particular outcome.

4. Your Responsibilities

To help us provide the most appropriate support, you agree to:

  • provide accurate and complete information;
  • disclose current medications, supplements and significant health conditions;
  • advise us if you are pregnant, breastfeeding or planning pregnancy;
  • inform us of allergies or sensitivities;
  • notify us if your health changes significantly;
  • ask questions if anything is unclear;
  • make your own informed healthcare decisions.

You understand that recommendations are based on the information available at the time of your consultation.

5. Individual Responses

Every person is unique.

Responses to complementary health approaches vary considerably between individuals.

Many factors—including genetics, lifestyle, nutrition, environment, stress, adherence to recommendations and existing medical conditions—may influence outcomes.

For this reason, Root Cause Wellness cannot guarantee any specific result.

Testimonials shared by other clients represent individual experiences only.

6. Existing Medical Care

Unless advised otherwise by your treating healthcare professional, you should continue taking prescribed medications and following medical advice.

You should not discontinue or alter prescribed treatment without consulting the healthcare professional responsible for your care.

7. Children’s Consultations

Where consultations involve children:

  • consent must be provided by a parent or legal guardian;
  • the adult attending confirms they have authority to make healthcare decisions for the child;
  • information provided must be accurate to the best of their knowledge.

Where appropriate, we may recommend referral to another healthcare provider.

8. Consultation Records

To provide continuity of care, we maintain confidential consultation records.
These records may include:

  • consultation notes;
  • health questionnaires;
  • remedy recommendations;
  • follow-up notes;
  • correspondence relevant to your care.

Your information is managed in accordance with our Privacy Policy.

9. Telehealth Consultations

Many consultations are conducted via telephone or video.

While we take reasonable steps to maintain privacy and confidentiality, electronic communication carries inherent risks.

By participating in telehealth consultations, you acknowledge and accept those risks.

10. Follow-Up Consultations

Healing is often a process rather than a single event.
Follow-up consultations allow us to:

  • review progress;
  • assess changes;
  • adjust recommendations where appropriate;
  • answer questions;
  • provide ongoing education.

Attendance at follow-up consultations is always your choice.

11. Communication Between Consultations

We welcome brief administrative questions between appointments.

Clinical questions that require assessment, interpretation or significant professional time may require a follow-up consultation.

This helps ensure recommendations remain accurate and appropriate.

12. Cancellation Policy

We understand that life happens.

If you need to cancel or reschedule your consultation, we ask that you provide as much notice as possible.

Cancellation terms are available on our booking page.

Failure to attend without notice may result in consultation fees being forfeited where permitted by Australian Consumer Law.

13. Payment

Payment is required at the time of booking unless otherwise agreed.

Consultation fees cover professional time, assessment, preparation and recommendations.

Product purchases are separate unless specifically included.

14. Limitation of Services

Root Cause Wellness reserves the right to decline or discontinue services where:

  • information provided is intentionally misleading;
  • abusive or threatening behaviour occurs;
  • inappropriate conduct occurs;
  • we believe another healthcare provider is better suited to assist;
  • continuing the practitioner-client relationship is no longer appropriate.

Where practical, we will recommend alternative avenues of support.

15. Limitation of Liability

To the fullest extent permitted by law, Root Cause Wellness is not liable for indirect, incidental or consequential loss arising from your participation in consultations or your use of our educational materials.

Nothing in this document excludes or limits any rights or remedies that cannot lawfully be excluded under Australian Consumer Law or any other applicable legislation.

Where liability cannot be excluded but may be limited, our liability is limited to the maximum extent permitted by law.

16. Acknowledgement of Informed Consent

By proceeding with a consultation, you confirm that:

  • you have read and understood these Consultation Terms;
  • you understand the nature and scope of complementary health consultations provided by Root Cause Wellness;
  • you have had the opportunity to ask questions before proceeding;
  • you understand that recommendations are educational and based on the information you provide;
  • you understand that no specific health outcome can be guaranteed;
  • you understand that complementary health services are not a substitute for emergency or appropriate medical care;
  • you freely consent to participate in consultations with Root Cause Wellness.
